Failure Mode and Effect Analysis (FMEA) Training Workshop
Overview
A one day workshop that can be provided on-site or at another appropriate venue.
For anyone involved in problem solving or identifying potential improvements.
The Failure Mode and Effect Analysis (FMEA) Training workshop consists of a series of theory sessions introducing each FMEA concept and its associated tools. Each concept theory session is supported by practical exercises that are completed before moving to the next concept. Two case studies will be used for the practical exercises. One is a simple case to demonstrate and practice the technique. The other is a real case that the participants will select from their work environment. Over the duration of the workshop, participants will apply the FMEA methodology to their real example, working together to produce an A3 report on their proposed treatment.
The anticipated outcomes for the workshop participants are;
- A working knowledge of the FMECA framework and the sequence of events required to conduct an effective FMECA
- An understanding of how to use the problem solving tools provided in the toolkit
- The ability to conduct an FMECA and assess its effectiveness.
Typical Agenda
- Overview of FMECA
- The FMECA Worksheet
- The 8 steps of FMECA
- 1 - Prepare – clarifying the problem
- Establishing Purpose of the FMECA
- Developing Criteria for assessing impact
- Risk Priority Number (RPN)
- Exercise in clarifying the problem drawing upon problem solving tools;
- 5Ws and 2 Hs
- Flowcharting / Process Mapping
- Brainstorming
- Team Voting
- Paired matrix evaluation
- 2 - Analyse potential Failure Modes
- Identifying failure modes at each process step
- 3 - Analyse potential Failure Effects
- Identifying consequences and severity at each process step
- 4 - Analyse probable causes
- Identifying potential cause and likelihood of occurrence
- Exercise in identifying Probable Cause drawing upon problem solving tools;
- 5 Whys
- Ishikawa Diagrams
- 5M Method
- 5 - Analyse control Framework
- Review the controls in place
- 6 - Prioritise potential Failure Modes
- Assessing which failure modes to address first
- Exercise in prioritising potential failure modes
- Pareto Analysis
- 7 - Develop action Plan
- Create a documented and structured plan to prevent or reduce the likelihood of an event occurring, minimise the consequence of the event occurring (or transfer the risk), increase the ability to detect the event and provide early warning and reduce uncertainty surrounding the event
- Exercise in developing an action plan
- A3 Reporting
- 8 - Re-Assess – Post Treatment
- Measure the effectiveness of the FMECA and review opportunities for further improvement and error proofing
- 1 - Prepare – clarifying the problem
